This notice explains the contact forms on the TannerMD launch website. This page is not a patient portal, medical-record upload service, or online appointment system.

What we collect

For care inquiries, we ask for your name, contact information, preferred contact method, and the state where you expect to receive care. For report-pilot and launch updates, we ask for your email and optional state. For privacy requests, we ask for your email to arrange follow-up. We also record the submission date, the form selected, and your consent. Please do not submit medical records or clinical details.

How we use it

Dr. Tanner, authorized associate providers, and authorized support staff use care requests to follow up about availability and next steps. Joining a pilot or updates list permits email about that selected topic. Requesting care does not automatically subscribe you to marketing. Submitting a request does not book an appointment or establish that care is available for your circumstances.

Storage and access

Submissions are stored on TannerMD's managed server, with administrative access restricted to authorized operators. The forms do not send your information to an AI service. This launch page uses no advertising pixels or third-party analytics. Infrastructure may process technical connection information for security and service operation.

Your choices

After joining an email list, the confirmation screen provides a personal unsubscribe link. Save it to stop TannerMD launch and pilot emails. Future update emails will include an unsubscribe link. To request access, correction, deletion, or help with your preferences, request privacy follow-up. We will verify your request before making changes to your information. Unsubscribing from updates does not cancel a care inquiry.

Retention

Contact information is retained while needed for your inquiry or the updates you requested, subject to applicable recordkeeping requirements. Unsubscribing stops the relevant subscription. Authorized staff can review correction and deletion requests; backups may retain information until their retention period ends.

Urgent needs

These forms are not monitored for urgent medical needs. For a medical emergency, call 911. Do not delay seeking care while waiting for a response.
